COMPOSITION:
Each tablet contains LAMIVUDINE 150mg/tablet ZIDOVUDINE U.S.P. 300mg/tablet.
PROPERTIES:
LAMIVUDINE and ZIDOVUDINE when co-administered demonstrate synergism
and have non-overlapping toxicity.
USES: Treatment
of HIV infections when therapy is warranted based on clinical and/or
immunological evidence of disease progression. Bivek is given twice
daily, providing an alternative regimen to LAMIVUDINE (150mg) twice
daily plus ZIDOVUDINE 600mg/day in 2 divided doses. This combination
drug form reduces capsule /tablet intake of these two drugs to 2 per
day, instead of up to 8. It therefore improves compliance. Mechanism
of action: ZIDOVUDINE is a thymidine analogue which interferes with
the HIV viral RNA dependent DNA polymerase resulting in inhibition
of viral replication, and LAMIVUDINE is first tiphosphorylated into
its active form. In this active form it inhibits HIV reverse transcription
via viral DNA chain termination. It also inhibits RNA- and DNA- dependent
DNA polymerase activators of reverse transcriptase. The monophosphate
form of LAMIVUDINE is incorporated into the viral DNA by hepatitis-b
virus polymerase, resulting in DNA chain termination. Dietary considerations:
may be taken without regards to food.
DOSAGE: children
over 12 years and adult: one tablet twice daily; adjust dosage in
renal failure.
MONITORING PARAMETERS: Monitor CBC and platelet count at least every
2 weeks, MCV, serum creatinine kinase, viral load and CD4. Observe
for appearance of opportunistic infections. Also monitor amylase,
bilirubin, liver enzymes, haematologic parameters, viral load and
CD4 count, and watch out for signs and symptoms of pancreatitis.
